How do you know its not actually going into gear but the transmission is bad internally?

To answer your question yes it can. You can feel no pressure from the shifter if the cable is broken, if it is just bad you might feel pressure. The best way to check it is put your car on a car lift and shift your transmission on the transmission itself. Side note an internal transmission problem can cause your transmission not to shift as well.
