Contact the plan administrator and request a withdrawal. Contact info for them is available on the quarterly statements they send. You will have to pay income tax on the withdrawal (they will automatically withhold 20% as a down-payment on the tax bill) and if you are under age 59-1/2 also a 10% early withdrawal penalty (unless the money is used for one of the exception purposes identified by the IRS, or unless you separated from service from your employer at age 55 or later).
