I am here in US on H1 B visa. I got married in Apr 2006 and my wife came here in Jun 2006. I am filing my tax return as a resident alien for 2006 and claiming my spouse as an exemption. I have few questions on W-7 Form as I am applying the ITIN for my wife.

There is column in W-7 form which says " Reason for submitting the W-7...

----Nonresident alien required to obtain ITIN to claim tax treaty benefit
----Nonresident alien filing a U.S. tax return
----U.S. resident alien (based on days present in the United States) filing a U.S. tax return
----Dependent of U.S. citizen/resident alien
----Spouse of U.S. citizen/resident alien
----Dependent/spouse of a nonresident alien holding a U.S. visa

Out of these options which box should I check?

And what copies of passport do we need to send along with W-7 form??
