Diet for creatinine high level

Hi ! Doctor,
We have recently done my dad's Blood Creatinine & BUN test it showing results as blood Creatinine is 3.0 & BUN is 85 . And our Doctor says its 50-75 % Kidney failure.
He has Diabeties since last 25 yrs & now facing the Fluctuation of Sugar & BP for lower to higher level.
Is that correct that , this is Kidney faulure of 50-75 % .
Please help me
