If his employer was aware of your husband's passing I would expect that they would have automatically sent out paperwork to the benficiary (presumably you). But JudyKayTee is correct - if you know who the administrator of the company's 401(k) plan is, you should contact them and ask how it should be handled. Your husband may have kept some old account statements which would provide contact info. If not - then call his company's HR department and ask hwo to proceed.
