How to apply foe a new pass port after the first one is lost

I had a passport that was issued in jaipur in 2003.then I moved to kerala in 2004. After getting married. I lost my passport during house shifting in kerala 2 years back.I don't know my passport no. and I don't know how to apply for a new one .
