I made my husband sign a conditional talaq valid for 6 months after he came out of rehab, that he cannot do drugs. If he admitted that he has used once since he is out, I know a talaq falls, but how many? Also, can I make that contract after my talaq fell null and void so that no more can fall as I did it initially thinking that he wouldn’t use and this would safe-guard him but his will-power is weak and I’m afraid that he will again before the 6 months is up.

After all is said and done, it is between your husband and Allah if he chooses to stay away from drugs. He must work at it, and he must want it. And Allah will give him strenth if he asks of it.

I do not believe divorcing him or using divorce as a safe guard is the best idea here for you or him. I strongly urge you to meet with your Imam, with your husband, and get some help with both your marriage, and your husbands addictions, as well as your fears as his wife.
