My girlfriend and I had sex multiple times last night and we used condoms on all of them. But on one of the sessions we had a few seconds of uprotected sex I don't believe I pre-cummed at all and I unrinated a good 3-4 times before the session and after the last session. She is worried and wants to take Plan-B, is it really needed?

I would take it. Any time there's sex (unprotected as well as protected), a pregnancy could be the result.

Yes, condoms are known to have a high failure rate. And any unprotected sex can be a higher risk. If you are going to have regular sex she really needs to be on an additional form of birth control with the condoms.

Be aware that plan B will get her cycle off for a month or two.
