What is the proper locations of rubber, metal, and nuts to fasten tank to bowl keeps leaking from bolts

The rubber washers go on under the bolts inside the tank, and the metal washers and nuts go under the bowl. The large rubber washer in the center of the tank go between the tank and the bowl.
