If you claim he is not the father, and fight the paternity claim, he may ask that you pay for the DNA test, or at worst he pay for his and you pay the child. But he may have to pay for the DNA test.

No if he is the father you will pay for your attorney and he will pay for his. If he does not ask for anything out of the ordinary, if he just asks for standard visits he may even ask that you pay his attorney, that will be up to the court.

The court will not decide who pays medicaid, I will assume you listed him as the father to them, but it will be their choice.
