I have been assisting in the care of a very close friend. He has recently had a major procedure. I am in a relationship with someone else. Now over the last couple of weeks I have started massaging my friends arms, legs, feet and hands. I do this for the circulation. With the problems that he has had circulation problems are a risk. Recently he has started interlacing his hands with mine as I massage his arm. He will even give my hand a slight squeeze. Could there be a hidden meaning to this? Could I be reading more into this than what is actually there?

Yes you could be, but only he would know for sure. Maybe he is learning to do it himself. The one to ask would be a more experienced fellow worker, or supervisor.
