What can I do if somebody forged my name on altered and changed documents

You could press charges for forgery.

That's the only answer we can give without knowing the nature of the documents.

Well some documents were altered, changed, & signed in my name without my permission even got it notarized what can I do about this

If the notary is required to have a bond in your state or country, make a claim on the bond.

This doesn't answer my question.

But if your signature was verified by a notary falsely you can bring them up to their licensing board.
