Formal letter of a cleaning proposal?

We use a proposal that when signed is a contract it states who we are, how long we have been in business, how we train our people, our business structure, what we specialize in, references, etc

Then we focus in on the particular bid; state net square footage to be cleaned, how many days per week, any special needs, time cleaning will start each day, security needs, any floor work, schedules for floor work, any lights to be left on when we leave, any interior doors to be locked, etc. Cost page breaking out monthly, quarterly charges and the billing information page.

And finally the signature page.
