Depending on the wiring system, there could be BX or metal conduit in the box and that my be grounded. Is the box metal or plastic? Is it a fan rated box? How old is the house? If this is a regular plastic ceiling box, please replace it with one rated for a fan or light fixture. The old boxes, particularly plastic will not hold the fan mount screws and the fan can fall out.

Neutral can be White or Gray. That's wired fine. The black and gray
Like above, is the box metal or plastic?
