Good luck finding one... you will have to actually call and ask them, use the phone book to find ones local enough to you as a list.

I'm not sure any want to advertise that because if they take one... they have to take many others. And if they advertised it they would flock there and few could afford that at the reimbursement rates they would get.

Do you have an insurance company that acts as your medicaid servicer? (ie: Anthem heathkeepers Plus, or OptimaCare) COmtact them and ask for a provider directory.

Or try looking it over the internet.

Your insurance provider has a list of Optometrists that accept your insurance. Contact them.
