I am going to my first and most likey all of my ob/gyn appointments alone. I am three weeks pregnant and the father lives five hours away. My first appointment is in a few days and I need to email him a list of questions to answer so I can ensure my baby gets the best prenatal care. This is my first child and I have no idea how to be prepared for the visit!

You'll want to know if his family has any history of mental illness (depression, bipolar, schizophrenia), alcoholism, autism, Alzheimer's, Parkinson's, Down's syndrome, heart disease, diabetes, stroke, epilepsy, muscular dystrophy, cerebral palsy, etc. -- all those major things that people suffer from during their lives. Even dental and vision (e.g. glaucoma) problems might be good to know about and have on file.

What did any close family members -- parents, grandparents, uncles, aunts, first cousins, siblings -- die of? And how old were they when they died? Stillbirths? Fatal car (or other) accidents?
