    In my birth certificate wrote my mothers nickname.how should change her name in my birth certificate?what should I do?

    Where are you located? How old are you? Who filled out the form incorrectly? Why do you need the birth certificate amended?

    If she is still alive and you have contact with her, you can ask her to go down to the social security office and first talk to them, and then ask them where to go about the birth certificate. They're a government office, they'll know what to do.

    This is a Law board and our answers here must be accurate according to such. SSA does not handle the issuance or correction of birth certificates.

    Typically in the United States, the OP would have to contact Vital Records in the county of their birth - however, we don't even know at this point if the OP is even a US resident.
