Don't want to change my sons surname just want to add mine on (his mums)

A birth certificate is a legal document and generally requires a court order to change. It would also, likely, require the permission of the father. If your son was just born and you want to correct the certificate you may be able to do that. Contact the agency that records birth certificates or the hospital where your son was born.
