    Hi
    I am in US from Sep 2008 to till 13th May 2010. I came back to India and would like to withdraw my 401K now. Presently I am in India and would like to know, what form need to be filled in and submit to my provider?

    Talk to your 401(k) plan administrator on how to process your request. You will of course have to pay US income taxes on the withdrawal, plus the 10% early withdrawal penalty.
