Proper credit for the health benefits:
Normally, court-ordered health insurance or benefits is over-and-above the monthly support payment required. Unless the child needs health care, and the custodial parent petitions the court to force the other parent to take care of it, the court wouldn't be involved. I don't understand why you need to bother the court about this.

You will need to prove changed circumstances, usually.
When the court kicked the paperwork back, did they explain why?
