My attorney needs me to sign a release for defense & pre medical history form.
It looks like this case is going to court was hoping for a settlement. I could not get the secretary to explain the forms meaning and why I now needed to sign them. A release of defense sounds bad and the other pre-medical release sounds like they are want my medical history records. How does the insurance and or attorneys obtain my complete medical history and where can I go to get the same records. Is there a limit as to how far back they go. What if I can't recall past physicians, is there some main data base that the insurance and attorneys use? Can I use that same data base.
    Thanks for any help...

USUALLY the medical releases are limited to that particular accident and any pre existing injuries.

You are paying an Attorney (this is probably a contingent matter). You need to ask these questions of that Attorney. If there is ANYTHING you don't understand - ask!

They can go back as far as the release allows them. If there is no time frame they can go back as for as they want.

As JudyKayTee said, it's not the secretary's job to advise you concerning the meaning of legal documents. That's your lawyer's job.

But, I would add, don't be concerned about letting your lawyer have too much medical information. It's in your interest that your lawyer know as much about your medical history as possible. You should assure yourself that he understands you don't want him to share anything without your express permission.
