Friend of the family, who is mentally disabled, just gave birth to a little girl. She wants to sign off rights, doctor and nurses are saying we only have to sign papers. Is that right

Nope... its not a puppy. It's a human baby.

Nothing happens without a court doing it.

Doctor and nurses are idiots. That is a legal nightmare - if she is mentally disabled, all it takes is one person to say she didn't understand the ramifications of the agreement she signed granting you custody. Then the kid gets ripped out of your home and probably placed in foster care until they can evaluate the mother and determine if she is a fit parent or not.

The only LEGAL way is to go through the courts. And of course, the mother is going to need legal representation to explain to her what she's doing.
