When having twins, do both babies have to be head down for a vaginal delivery or just the lowest baby?

Often times, when baby A is head down and then born, baby B will then position head down prior to birth... far more room now for baby B!

It can happen, however, that after baby A is born, if baby B does not turn to a favorable position a cesarean birth will be necessary for baby B.
