I was bleeding very heavy then a very large blood clot came out I became dizzy and my ears were ringing. Two weeks later I'm spotting and have sore nipples. What's wrong? I did not see a doctor I have no medical coverage and cannot afford to pay.

Impossible to know. First month spontaneous miscarriages are common, however, and it may very well have been, even with spotting later.
