The condom fell off when I was pulling out and she claims to be very good about taking her birth control. Is there any chance she could be pregnant?

Anytime you have sex there is always some risk of pregnancy so yes, there is a chance she could become pregnant. But if she really does take her birth control correctly then there's at least much less of a chance of her being pregnant.
