Can we file a small claim in small claims court for medical bills if my wife's ex husband won't pay his percentage even though it was in domestic court when they got divorced.

If someone involved in a divorce is not following through on what they agreed to in a divorce settlement, then you start by asking the divorce court to cite them for contempt.

And I will add that SHE can file for contempt in the Court that issued the Order. You have no legal standing.
