The bank goes by one name, and it demands another name be used for court & levy actions. Is an amendment a possibility, and how do I draft it?
What document is it that you want to amend?
Apparently a Writ of Garnishment.
Reason I ask is that OP said "court & levy actions". Makes me wonder if the bank is the garnishee or the defendant.
If it's a writ of garnishment, you could simply re-file an application for the writ, replacing the name of the bank with the name the bank wants you to use. Alternatively, if it fails to respond to the writ, get the court to make it respond, and pay your attorney fees for dragging it's feet.
My best guess at what you're saying is that a bank was bought out and is now operating under another name, hence the use of two names. For example: J.P. Morgan Chase Bank was formerly Bank One, so either name might be used.
If that's what you're inquiring about, I fail to see why this is even an issue.
Are you trying to levy against the bank, or are you employed by the bank and are try to place a levy against someone else?

I would assume you merely do a new garnishment with the name they require on the document.
