If this is the bio father and no other father was listed this should be a simple matter. Just contact the agency that issues birth certificates and ask for the forms to make the change.

You cannot add a father's name on BC if:
1.He was not married to you.
2.He was not adjudicated as the father
3 He has never signed Acknowledgment of paternity.

As long as one of those items is true can the father then be added? Or do all three have to exist?
