Pain is relative and everyone has different pain tolerances. I have a very high pain tolerance and for me, personally, it didn't hurt more than getting my lobes done. I've heard a lot of other people say the same. Maybe search the forum for other personal experiances, there's plenty of similar threads.
They will if you don't take care of them. You need to be diligent about cleansing and turning the stud and making sure the piercing doesn't crust up. Also, avoid sleeping on it, that will just irritate it.
Cartilage takes a lot longer to heal than just skin, usually 8-10 weeks.
Definitely a needle. Go to a professional at a tattoo parlor/peircing shop. Guns are less acurate and can damage your cartilage. Also, a lot of times guns don't get disinfected after being used, whereas needles get tossed.
Here's a great site to look at before making your decision: Ear Cartilage Aftercare
Good luck!
