I received an ITIN number 4 years ago and never used it. Now 4 years later I tried using it and it's saying that the ITIN doesn't exist. Copuld it have been revoked? And if so what do I do to get another one or get the old one reinstated?

Shouldn't be.. simply contact IRS.
