Need to know the exact format to file a motion in Dekalb county superior court to request the judge to allow my 21 year old daughter to receive child support arrearage payments instead of my EX.
Someone help me please an example motion to get me started!!

The money you pay in child support is to offset the costs of raising your child - it isn't spending money for your daughter. That's why your ex receives the money, not your daughter. Your daughter didn't pay for her own food, clothing and shelter while she was a minor. So you can complain all you want; I doubt any judge is going to grant your motion.

Legality aside, I find your username to be tacky and quite obvious as to what the issue is - you don't care for your ex and don't want her to have "your" money. Had you paid it on time, you wouldn't be in arrears and she wouldn't be getting anything more from you.
