I need to do a power of attorney, in case my parent, become unable to make decisions, in all matters like health and financial decisions.

Which type ofPower of Attorney form do I use, because I hear that is at least 2 types of Power of Attorney Forms.

Any tips will be appreciated.

This will suffice:

General Power of Attorney Form

Are your parents OK with this? You can do a conditional POA that goes into affect only if they are unable to make their own decisions.

Except that particular one is, oh, so 20th Century. (The blank for the year should be "20__"). :)

If your parent so truly not capable of making any decisions he may not be able to agree to a POA. You might have to petition the courts to declare him incompetent. To be sure we'd need to know more about his conditon. There is a fine line between an elderly parent making bad decisions and not capable of making decisions. Can you tell us more about your dad's situation?

The OP wrote " in case my parent, become unable to make decisions". Future tense, clearly.

My mom is fine now, this is just to be prepared in case she ends up in a coma, or due to some illness or incident would need a power of attorney, which form would give me full control to take care of everything like hospital decisions, financial, etc?
