For your NJ statw income tax return use the NJ State wages number. The difference between the federal and sate nyumbers is because some of your deductions are "pre-tax" for federal purposes but not for NJ (your contribution to medical and dental, for example).
Your 414H contributions should alreadu be excluded from the amounts reported in both your federal and state wages on your W2, so no - do not deduct them a second time. For reference, see: http://www.state.nj.us/treasury/omb/...pdf/w2info.pdf
