Why is it when my boyfriend is about ready to ejaculate, the back of his head hurts really bad?

Is this new? Has he experienced this before? Does it matter the position? Does it happen with self stimulation? Exercise? History of tension-type headaches?

Blood pressure will increase during sex and muscles will contract, which can impede blood flow...

Again... is this new? Happens every time?
