I want to replace my tub spout. The nipple is a 1/2" copper pipe with 3/4" threads soldered on the end, giving it a total clearance of 3.5" to the end of the pipe. I can't seem to find any replacement spouts that will fit.

Should I cut the threads off and attach a slip-on spout or is there a threaded extension piece allowing a spout with a larger required clearance to screw on?

There are basically two kinds of tub spouts with diverters... the threaded kind and the slip on kind (see pictures below). The slip on type spout is missing the diverter in this picture but presents the idea for you to see it.

To install a slip type make sure to cut the pipe to the correct size see the images below, de burr the end of the copper pipe, use a little silicone GREASE to lubricate the new Oring just prior to installation so the new Oring doesn't bind and break, the Oring is inside the tub spout, good luck.

Don't waste your time trying to find the spout that will fit. As you said: cut off the threads. Install new Slip-On spout following John's instructions.
