My daughter is 15yrs old, I've worked 2jobs while I was pregnant & her father in jail. I left him when she was 1 1/2 due to his drug problem. All this time went by, he has done nothing for her. I did not file for support since he was worthless in keeping a job. He was in jail 12 times for drugs & he tried to make amends with her when she was13. Now she is old enuf to make decisions on her own, & after many times of him promising things & not coming through, not calling her like he should & not seeing her at all. We gave him the benefit of the doubt when she was 13 to make up for past... but never did he change, now she has agreed to remove him from her birth certificate & have nothing to do with him @ all. How can I go about this procedure?

As far as I know, you can't. The birth certificate just says who donated the genetics which made her, not who was a good or bad parent.
