    a) the country that issued the passport
    b) the number of the passport
    c) the expiration date of the passport

You have to send the W7 to
    Internal Revenue Service
    ITIN Operation
    P.O. Box 149342
    Austin, TX 78714-9342
Together with a copy of the return you need the ITIN for and all necessary documents.
You can file your return without waiting for the ITIN. In the filed for SSN you write "ITIN pending" and attach a copy of the W7 to the return.

BTW, if you send a notarized copy of the passport, that should be enough (see instructions for W7)

    A subsequent change in policy:

For a child, attach a photocopy of the child's VISA as well as the photocopy of the passport. The IRS wants proof that the child spent some time in the United States in 2009.
