The mother of my baby thought that she was her live in boyfriends,so when born she gave her that mans last name. We have since learned that the baby is actually mine. I want her to have my last name. How can I change it?

Go to court. Prove you're the legal father through a DNA test. Then start proceedings after that for support, visitation and name change.
