The main things that need to be done is list the circumstances of the out of country visit, where ( region ) you are visiting, how long it is expected to last. Also a good idea to list any health insurance on the document for the child. This will have to be noterized by the parent giving permission. Also if there are any restrictions or contact numbers you may wish to put that on there also.
