    Who is my doctor ?
    This is a strange question but I'm hoping someone will have the answer. My mother is elderly. Recently she visited a specialist for an eye problem and the doctor recommended minor surgery to fix her eye, but there is one problem: she lost her paperwork and can't remember the doctor's name or address. We know she was scheduled to have this done on December 31 and that's all we know. We're trying to figure out the doctor's name, address and phone number. Is this information usually kept in some kind of public record that we can research ? Are there other ways to identify this doctor ? Thanks.

    How about the doctor's contact info.
    Well, in that case I don't think it is still possible for you to find the doctor. Here in our country?it is very easy to identify the person if you can still remember he's face! <ha ha! Hope it make sense>

    Contact info lost too ! Maybe it's time for a classified ad haha. Any other ideas please ?

    Hello moon:

    There is NO public information about this doctor and your mothers appointment.

    The best thing to do, is make another appointment with another eye surgeon.

    Or, if the FIRST eye surgeon is worth his salt, his office will CALL prior to the appointment to confirm it, and to give your mom last minute medical instructions in preparation for the surgery...

    If not and he doesn't, there are PLENTY of eye doctors out there. Ain't no big deal.

    Assuming your mother has health insurance, contact the carrier and ask. The carrier would have been billed by the Physician and would have the name. The carrier very probably will not give you the info but will give it to your mother.

    Either call or go back to the first eye specialist. They will definitely have in your mom's medical records that a referral was made to a particular doctor. Then, when you have that doctor's name, call up and see if this is indeed the eye doctor your mother has an appointment with.

    Or you could call all the eye doctors in your city and see if they have your mother scheduled for an appointment in Dec.

    Also she can call the medicare payment department and find out who was recently paid
