    Regarding name
    Hi, I am a Govt. employee, married. I want to apply for passport. My identity is S. Mamatha Sharma in SSLC certificate and is known by this name in Government Service Register. My husband's name is A.S. Srinivas. I want to know whether I have to apply for passport in my husband's name or my father's surname.

    If you are using your husband's name there should be a place to write your maiden name on the passport application. If not, well just use the name you were born with. The one that shows on YOUR papers not HIS... Check on Passport Offices websites, you can ask them directly (call early in the morning)

    By law, you can choose whether to use your maiden name or husband's name. My opinion, it's much easier to go with your maiden name. Name change means a lot of $$ and time.

    You're right. I was answering the Canadian way:)

    Here is an excerpt from Passport Office Canada

    Names
    All of your documentation, including your passport, should be issued under the same name. If you have changed the name on your other identification documents, your passport should reflect the same name. Applicants who have changed their name cannot have a passport issued in their new name through the Simplified Passport Renewal Process.

    Married surnames
    To obtain a passport in your married surname, you must submit:

    An application form completed in full;
    Proof of Canadian citizenship;
    Supporting identification in your married name;
    Two photos, one of which is signed by the guarantor;
    The required fees; and
    Any valid Canadian travel document, if applicable.
    You may be requested to submit your marriage certificate in support of your request to have your passport issued in your married surname.

    You may no longer add your married name as an observation on a passport. If you wish to obtain a passport in your new name, you must apply for a new passport.

    I believe following the rules is the simplest way and saves a lot of headaches
