    Regarding photocopies for ITIN application
    Hi,

    I am in H4 visa with my husband in US. He wants to apply ITIN for me. Should I attach the photocopy of all my passport pages? Do I need to attach my husband's passport copies? What are the other documents that has to be attached? Where can I get my photocopies notarised?

    If your husband is applying for you I assume he already has his number.

    Download the W-7 here:
    http://www.irs.gov/pub/irs-pdf/fw7.pdf
    You can fill it in on your computer.

    You check box (e) filling in his name and taxpayer ID number. Enter all your personal information.
    On line 6d check the passport box and fill in the information below that.
    On 6e check NO.

    SIGN AND DATE IT!

    Now you go to the office that issued you your passport and have them certify the photocopy of your passport as a true copy. That certification should take the form of the internationally recognized "apostille" attached.
