Where can I find form- notice to pay or quit in PA

I don't know where you would find such a form. It's not uniform at all and it's not required to be "official". All it needs to say is the basics... You can prepare it yourself by hand, and send it by certified mail... It could be real simple, like:

Dear Tenant:

Pursuant to PA lanlord tenant law, this is your (3) (5) day notice to pay rent or quit... If I don't have your rent by noon on Sept. XXX, 2009, a lawsuit will be filed for your eviction.

Your landlord.
