Personally I don't like the iron on method because they can crack and peel.

Is there any way to keep it on permanently, no matter how many times you wash it?

I think the trick may be in ironing it and getting it into the fabric better. Like probably not too hot of an iron and iron it long enough that it adheres good.

You can also use DecoArt paint they also have a medium you mix with the paint to keep it soft and so it won't crack. You can get the paint at joann fabrics, michaels, hobby lobby, places like this. I ironed it after it dryed. DecoArt has its own web site. The bottles are small so it won't cost a lot. I've used this myself. Even without the fabric medium that stuff does not come off. It may fade in time but not disappear.
