How do I Format this letter. Check punctuation
This is an evaluation of a 46 year old woman, weighing 116 Pd's, standing at 5 ft. 3"s tall. She is postmenopausal, small stature and has a family history of osteoporosis. Current therapy includes; regular weight bearing exercises, calcium supplementation, vitamin D and hormonal replacement therapy. The lumbar image is of good quality. The patient has normal bone mass. Regular testing is not necessary for the next 5 years, if she continues with the above treatment program.
Please do not hesitate to contact me, if you have additional questions about this evaluation.

With Current therapies include I would use : instead of ; or not use any punctuation.

With Please do not hesitate to contact me I think you can get away with no comma. In fact I think it would be better without one.
