Ask Me Help Desk - Extreme Difficulty Urinating

When I am urinating I often need to be on the toilet for 5 minutes or sometimes longer. I do have a split stream while peeing. Sometimes I have extra difficulty and no matter how my bladder feels (even when it feels like it can just explode its so full) I have to take a lot of time to do it. When I tense my PT muscle or PC I'm not sure what the exact name for it is but I tense it up then for some reason I can't help but relax it after 5 seconds or so and try again and again until I get closer and closer and then finally I feel the urine come and then I begin peeing... I think it may be something to do with my weak PT muscle but it wasn't a problem until 2 or 3 years ago and its been getting worse and worse. I am on a bladder relaxant and my anti-depressant (Celexa if that makes any difference) and even after seeing the urologist he did do a scope in my urethra and said it was clear and I still have extreme difficulty urinating along with a bladder scan as well. At first he asked if I had a split stream I said no but now that I think about it, I may have when he asked. Please help!
This problem of mine has really gotten in the way of me living my life... I have 45 minute classes and between every single class I have the urge to go pee and the 20 minute bus ride before and after I have to go pee as well... help please

How old are you? Do you have a history of urinary infections? How much do you pee each time you go? Are you sexually active? Do you have any STDs? BPH? Are you constipated? (do you force on the toilet?) Is your depression the result of this condition or something else? Do you smoke pot? If so, how much and how often? Same for alcohol. Are you currently exposed to toxic chemicals? And, what is your instinct about what is causing this?

Answer those questions and some insight can emerge. The split stream can mean that your urethra is inflamed. Your doc should have told you if such is the case, and whether you have BPH. Did he test for infections?

Take a Google at Zyflammend, pomegranate, Turmeric (curcumin), Vitamin D3, and D-Mannose. Do your own research on these items. When you decide what is appropriate for you, let me know, and I will recommend inexpensive sources.

I am 19. I don't have a history of urinary infections. I pee about 10 seconds worth of basically a full strength stream when I'm not forcing it anyway. No I am not sexually active, no stds, I had a prostate exam a few months ago it was normal. I am constipated sometimes. My depression is in part of this but mostly college stress and relationship trouble and basically anxiety. I don't smoke pot. I do not drink except for a beer every once in a while. I wash dishes at a restaurant so cleaning agents maybe... but I never had bleach in my eye or anything serious like that happen. And I will read up on those thank you

Since constipation is part of the picture, I would add magnesium to the list.

At this point, let''s be clear: I can't diagnose or prescribe anything. I am not a doctor or licensed medical professional. Anything I tell you and any line of thought I pursue should be verified with your urologist, and anything you do to improve your condition should be coordinated with him. I am simply a person with experience and am stating what I would do if I had what you have: extreme difficulty urinating, without evidence of NSU (non-specific urethritis) or prostate involvement.

That said, the first thing that pops up is constipation coupled with anxiety and depression. One should consider a physical/mechanical side and a psychological/emotional side to your situation. According to your report, your doc didn't point to anything physical, at least not anything obvious. You should ask him again.

Since you are not sexually active, I would also ask him about seminal vesiculitis. How often do you ejaculate? If you don't have wet dreams or masturbate, I would look into the possibility of a chronic infection somewhere in your seminal plumbing. My guess is that clearing things out could possibly be useful.

The constipation might be a factor. If you take up to 1 gram of chelated magnesium with a glass of water before bedtime, rumor has it that you will get soft, easy stools the next morning. People tell me that one starts with 250mg and adds 250 at a time until arriving at 1000mg, which is one gram. Do your homework on magnesium and make your own decision.

One is looking for a soft stool, but not diarrhea. The purpose is to avoid straining.

Then. I'm wondering about your anxiety and depression. School can be depressing, as well as anxiety producing. Being 19 can, too. Being any age for that matter, unless you can let go and take life in stride, can be difficult. What do you understand about how your emotional state plays into your urinary issue? Could it just be the chronic constriction of the muscles of your pelvic floor? If I were having what you describe, I'd explore that premise-- that it's a chronic self-holding habit that is behind all this.

When you have researched the above and talked with your doc about it, let me know what conclusions you have come to. Other people have figured these kinds of problems out, so chances are that you can, too. You might need some psychotherapeudic help, but that's a small price to pay for getting this under control. Wow, taoplr. Just... wow.

I had and still have a minor problem urinating because of my anxiety. Its because I expected it to just happen, that when it didn't, I tensed. Its not something you can really control. But understand and accept it, and its not really a problem anymore. Takes me about 30 seconds to feel comfortable, and then I don't have a problem. If there's a lot of noise, someone watching me, or people passing behind me, it can take longer then 30 seconds before I even start.

Just thought id offer my experience.

Don't rush it.
