Im on a dependent visa and my wife is the work permit holder. Ive been in this country nearly five years now. My wife applied for permanent residecy but she did not include me. We are not yet divorce. Can I apply for my own permanent residency?

Not on that application. If she is the principal applicant and you are a dependent, there is no law requiring that she include you.
