When my son was born in July of 2008 I gave him his father's last name. His father did sign his because but when I got it in the mail his name was listed as unknow. We later found out by a dna test that he was not his real father and now I want to change his last name to mine and do not know how to go about that. Can anyone help me?

You find the real father and get his consent to change the name.

Who's name was listed as unknown ? Did he sign an aknowlgement of paternity ?

The guy we thought was the father. And no he did not sign an aknowlgement of paternity.
