How do I wire a fireplace blower fan to run when the thermal disc heats up, and not continuously as it does now. (110V wired directly to the fan)

All you have to do is cut one of the wires going to the fan motor, then use one of the wires to go to your disc and the other wire will be tied to the second wire coming from the disc.
