I am applying for an ITIN for my spouse. So when I mail my return along with the W7 form, should I staple the W7 Form to my 1040 EZ or keep it separate?

I am also including the First Year Choice Statement. Should this statemnt be stapled to the 1040 EZ. In what order should I place these documents and should they be stapled to any particular document or not

Documents I am mailing with 1040 EZ are W7 form and First Choice Statement.

I am filing an extension for PA state as my spouse does not have an ITIN. When I did the PA State return, I owe the state $1. Along with my extesion, should I mail in the check for $1?

Your tax return should be on the top and then attach other documents.

Thanks :-). I assume all the other documents are stapled to the 1040 EZ.
